In this letter, Rachmaninoff asks Mandrovsky (his secretary) to visit a doctor since Sonia (SR's granddaughter) wrote to him about Mandrovsky's bad physical condition, and specifically, a cough.  SR suggests that Mandrovsky should smoke less.  At the end of the letter SR says that he will cover all of Mandrovsky's medical expenses.
